We are a centre certified to run the MOXO test — a computer-based continuous performance test (CPT) that objectively measures attention, reaction time, impulsivity and hyperactivity, supporting the assessment of ADHD.

Dear Patients,
We wish to inform you that on 13 October 2026 we detected unauthorized access to the account of one of our clinic’s doctors in the Medfile system. Our findings so far indicate that it resulted from a phishing attack, a fraud aimed at gaining access to the account. The unauthorized person used the account to issue prescriptions without the doctor’s authorization.
The incident lasted about 30 minutes. Once it was detected, the unauthorized access was blocked immediately. The matter was reported to the system operator, the police and the Polish data protection authority (UODO).
At this time, we have no grounds to believe that our patients’ personal data were leaked. If you have any concerns, we will provide further information at kontakt@poradniaforma.pl
